§ 111.49. REPORTING REQUIREMENTS.

  • Each holder of a permit issued hereunder shall be required to report to the St. Joseph County Sheriff’s Department, upon forms approved by that Department, any change in the names and addresses of persons authorized to sell drug-related paraphernalia at the addresses for which the permit is issued within 48 hours immediately following the change. In the event the change includes the addition of one or more individuals authorized to sell drug-related paraphernalia, the report shall be accompanied by an affidavit from the person or persons that he or she has never been convicted of a drug-related offense.
